You get both of them. You can tell the hem tool is early based on how short it is. It has a wooden knob to adjust the height and it still works very easily. It measures 9 inches tall by 3 inches wide by 2 1/2 inches deep. The old stitched pin cushion measures 4 1/2 inches tall by 3 inches wide by 1 1/2 inches deep. Really a nice pairing.
